Health and Welfare Benefit Offering
McGuffin Creative Group offers medical, dental, vision, short- and long-term disability, and term life insurance to all W2 employees working an average of 30 or more hours per week. We use a third party, Gallagher, that manages all of our health and welfare benefits. They are located at 1111 W. 22nd Street #620, Oak Brook, IL 60523.

Our Gallagher contact is Susan Special who can be contacted at susan_special@ajg.com or 630-353-2362. As a new employee, Susan will provide you with your Benefit Orientation and assist you in the enrollment process. In addition, all MCG employees have access to an Employee Advocate (EA). The advocate’s role (EA) is to assist you with any benefit issues that you may encounter and answer benefit questions that arise. Our assigned EA (Employee Advocate) is Julie Wall. She can be contacted at julie_wall@ajg.com or 630-353-2336. Due to privacy laws, there may be times when a HIPAA form will be required in order for Julie to discuss your claims with the carrier and she will notify you if applicable. The form will simply require your signature to allow Julie to speak with the carriers on your behalf.

Waiting Period
Benefits are effective on the 1st of the month following 30 days of employment. Employees hired for project work lasting for three months or less are not benefit eligible. However, at day 90, if the project is expected to continue, benefits will be offered and, if elected, will begin on the 1st of the month following 30 days from the eligibility date.

Medical Insurance
McGuffin Creative Group understands that each family has different medical needs and financial capabilities.
Therefore, we offer three different BlueCross BlueShield plans. By offering three different medical plans, McGuffin provides you with the ability to choose what is best for you and your family. We are committed to supporting you in the cost of healthcare by contributing to the cost of premiums of whichever medical plan you choose at the following rate:
Employee Only – 70/30
Employee & Spouse – 60/40
Employee & Child(ren) – 60/40
Employee & Family – 55/45

Dental Insurance
McGuffin Creative Group offers a PPO dental plan through Sun Life.
In this PPO plan, staying in network affords you with higher reimbursement percentages, a higher annual maximum and a potential rollover if certain criteria are met. Please refer to your benefits booklet or contact Affiliated Benefit Consultants for more specifics.

Vision Insurance
McGuffin Creative Group offers a voluntary vision plan through EyeMed. Premiums are deducted from your paycheck.

Pet Insurance
For your furry, scaly, squishy loved ones we offer two kinds of pet insurance, each has its own benefits and treats.
Wishbone, A pet health insurance that covers the cost of veterinary care if your pet experiences an accident or illness. And Total Pet Plan, which bundles the best brands in pet care to cover everything your pets need at one low payroll deduct rate.

401K Plan
To help you plan for your future, we have a 401K plan that you are eligible for after one year of full-time work.
McGuffin matches 3.5% if you contribute 6% of your salary. You’re entirely vested after two years in the plan. Our plan is held at John Hancock and managed by OakBrook Financial Group. You can review your contribution info in Paylocity as well as on John Hancock’s website.

Life Insurance
Full-time employees are eligible for and are automatically enrolled in a group term life insurance program.
Enrollees may designate or change the beneficiary for this policy at any time. The company pays the premium for this program. The face value of this benefit is equal to one times the employee’s annual salary. For details, please refer to the summary plan document.

Supplemental Life Insurance
Full-time employees are eligible to purchase a supplemental life insurance for themselves at group rates.
Supplemental life insurance is a voluntary benefit and is employee-specific. Enrollees should refer to the summary plan document for eligibility requirements, plan limitations and additional information.

Short Term Disability
A short-term disability plan is provided for eligible full-time employees. Please refer to the Short Term Disability summary plan description for an explanation of the plan benefits and limitations.

Worker’s Compensation
Provided by McGuffin through the Hartford Casualty Insurance Company, you are covered by workers’ compensation insurance. If you are injured or become injured on the job, you must immediately report the injury or illness to your manager.
